Tired of completing patient forms? We’ve invested in better technology to help with this. Check-In and complete your patient information from your computer or mobile device with Phreesia™ Pre-Visit. Once your appointment is scheduled you will receive a text or email to complete your Pre-Registration Packet.
On the day of your appointment, please arrive 15 minutes prior to your scheduled appointment time. Please remember to bring a copy of your insurance card(s), your ID, and a list of your medications. If you are unable to keep your appointment, please call our office to reschedule at least 24 hours prior to your appointment time.
